The purpose of this survey is to ascertain understanding of the Vaccine Adverse Event Reporting System (VAERS) by healthcare workers. The data will be used to refine the educational programs on VAERS, to facilitate appropriate VAERS reporting, and promote vaccine science and safety. This survey is being conducted by Ashley Grogg MSN-RN. Results and educational information may be found on VAERSproject.com
Your participation in this survey is voluntary. You may choose not to participate. If you decide to participate, you may withdraw at any time. If you decide not to participate in this survey or if you withdraw from participating, you will not be penalized.
This is an online survey that will assess your knowledge of the VAERS system and will take approximately 5 minutes to complete. Your responses will be confidential, and we do not collect identifying information such as your name, date of birth, or employer. If you choose to provide your email address to receive updates on the research, it will be stored separately from your results. Your email address will not be shared, sold, or distributed. If you prefer, you can request updates by emailing Ashley Grogg at info@vaersproject.com.
We will do our best to keep your information confidential. All data is stored in a password-protected electronic format. To help protect your confidentiality, the survey will not contain information that will personally identify you. Individual responses will not be shared or made public. Aggregate data will be analyzed by Ashley Grogg and reports of aggregate data may be shared publicly.
There are no known risks associated with participating in this survey. The benefit to participating in this survey would include satisfaction in supporting educational opportunities on VAERS and the future of vaccine science & safety. The results of this study will be used for the development of educational programs for healthcare professionals.

However, this research qualifies for an exemption from IRB review because it meets the requirements of Category 2: Research that includes only interactions involving educational tests, survey procedures, interview procedures, or observation of public behavior.
