Rocket's 2021 Summer Virtual Games Team Event Volunteers

**  READ THIS FIRST **

The Rocket's are planning a team event at High Range Park, 1901 Loma Colorado Blvd NE, on Saturday, June 5th from 8:00 am to 10:00 am.  We will have scoring stations for some of the 2021 Summer VG events, and we need volunteers to help with the set-up, check in, scoring stations,  escorting the athlete's to the proper scoring stations and tear down.  If you would like to help out, you need to fill out this questionnaire.  Due to current restrictions, we must limit the number of participants in each group at any given time. To do this, we have divided the event into 30-minute time slots.  In this questionnaire, you must identify which time slots you would like to help with.  If you have information about coflicts or concerns, please put them in the comments at the end.

This questionnaire is divided into three sections: contact information, participation information and questions/comments.

Instructions for filling out this questionnaire:

(1)  Fill out the questionnaire for each volunteer.  If you have more than one person that will be volunteering, fill out a seperate questionnaire for each one.

(2) Most questions require a response.  Fill out all required questions and complete the questionnaire by clicking "Exit" on the last page.

(3) You can back up and review or change responses before finishing the questionnaire.  Once you exit, you cannot change responses.
