The Care department at Lifegate Church is responsible for overseeing the visitation of those who are in the hospital or in a rehabilitation facility. In order to ensure that we are meeting our goals in caring for people, we have developed a brief survey which we will begin sending to those we have responded to. You have been included in this initial survey because you or another member of your family were in the hospital or a rehab facility during the first half of this year. Therefore, if you would please take a few minutes to complete this survey, it would be most helpful to us is discovering how we might improve our care.
