The Care department at Lifegate Church is responsible for ensuring that those who are in the hospital or a rehabilitation facility receive regular visits during this time in their life. Our records show that during the month of August you or a family member received such a visit from someone on our Visitation Team. To help us ensure that we are serving our Lifegate Family well; therefore, I am sending you this brief survey regarding the visits you received and would ask that you take a few minutes to fill it out. This will help me to not only know that you have been cared for well, but also to discover if there is any way we can serve you and others better. All responses are confidential.
