Please rate your level of agreement with each statement below using the 5-point Likert scale, and add an optional comment if you would like to suggest a modification. If a statement falls outside your area of clinical expertise, please select “Unable to comment” rather than a forced rating — these responses are excluded from the consensus calculation.
How Q1 relates to Q2–Q5: Q1 asks you to rate the complete revised Statement 2.1 as a whole. Q2 through Q5 then isolate the four specific clauses that were the focus of Round 2 discussion, so that if Q1 does not reach threshold, we can identify precisely which element requires further revision. Please rate Q1 based on the statement in its entirety, and Q2–5 independently on their own merits — even if your view on a sub-clause differs from your overall Q1 rating.
